At Sanders & Associates, we are happy to answer your questions about counseling services at the time you call. When we schedule your first appointment, we will collect your insurance information and offer to check your benefits.
At your first visit, you will complete new client paperwork forms. We understand that your time is precious, that's why we have included our new patient paperwork forms on our website for you to complete prior to the visit. You can download the forms and fax them to our office at 708-234-7448.
